How anti-wrinkle injections work — the muscle-and-skin mechanism, how dosing is determined, what to expect from the treatment, and what good results look like.

Anti-wrinkle injections — known by several brand names that Australian advertising rules don’t permit us to publish — work by temporarily relaxing the specific facial muscles that produce dynamic lines.
This guide explains the mechanism in plain English, why dosing matters, what good results look like, and what they don’t.
The mechanism in three steps
- Injection — small doses (units) delivered with a fine needle directly into the target muscle
- Local effect — the active molecule blocks the nerve signal that tells the muscle to contract
- Result — the muscle stops contracting, the overlying skin stops creasing, and over weeks the dynamic lines soften visibly
The effect is local — only the injected muscle is affected. Adjacent muscles continue normal function unless they’re injected too.
Onset and duration
- Day 0–3: no visible change yet
- Day 4–7: gradual softening starts
- Week 2–3: full effect; most clinics review at the 2-week mark
- Months 1–4: results steady, gradually wearing off
- Months 4–6: muscle returns to normal function; treatment cycle restarts
Some patients metabolise faster — high-metabolism individuals, regular gym-goers, very active facial expression patterns. Some metabolise slower. Consistent treatment over time conditions the muscle to remain relaxed for slightly longer between treatments.
What it treats
- Dynamic lines — lines that appear when you move (frown, raise eyebrows, smile, squint)
- Hyperactive muscles — frown lines, masseters causing teeth-grinding, lip-line muscle activity
What it doesn’t treat directly:
- Static lines — lines etched into the skin from years of muscle activity that remain visible at rest. These need a combination approach (anti-wrinkle to stop further deepening, plus skin needling or peels for surface improvement, sometimes plus a small filler for the deepest lines).
- Volume loss — separate problem, addressed with dermal fillers.
- Pigmentation, vascular, or surface concerns — addressed with skin and laser treatments.
Why dosing matters
Anti-wrinkle is dosed in units. The unit count determines how much muscle relaxation you get and how long it lasts.
- Under-dosed: lines soften only partially, effect wears off in 2–3 months instead of 3–4
- Correctly dosed: full softening, 3–4 month duration, natural facial movement preserved
- Over-dosed: “frozen” appearance, slower facial expression, sometimes asymmetric eyebrow drop or eyelid heaviness
The right dose for you depends on your specific muscle activity. A skilled Adelaide injector starts conservatively and adds at the 2-week review if more is needed — better than over-dosing and waiting 3 months for it to wear off.
What good results look like
Natural anti-wrinkle results are characterised by:
- Dynamic lines softened, not eliminated entirely

- Eyebrow position maintained (not “Spock-brow” arched)
- Forehead lines reduced but not artificially smooth
- No drooping eyelids or asymmetric brow position
